USS Thresher (SSN-593) was the lead ship of her class and the first purpose-built deep-diving nuclear attack submarine. She represented a generational leap in submarine design, combining a new hull form, advanced sonar, and the ability to operate at depths that gave her a decisive tactical advantage over Soviet submarines. Lost April 10, 1963 during deep-diving tests approximately 220 miles east of Cape Cod. All 129 personnel aboard — 112 crew and 17 civilian technicians — perished in the Navy's worst submarine disaster. Thresher's loss led directly to the creation of the SUBSAFE program, the rigorous quality assurance system that has governed submarine construction and maintenance ever since.
