USS Vincennes (CG-49)
USS Vincennes (CG-49): July 3, 1988
Commissioned on 6 July 1985, USS Vincennes is the most studied warship in the history of naval human factors research. On 3 July 1988, operating in the Strait of Hormuz during the tanker war, Vincennes engaged what her combat information center assessed as an attacking Iranian F-14 fighter. The aircraft was Iran Air Flight 655, an Airbus A300 commercial airliner climbing through 12,000 feet on a scheduled passenger flight from Bandar Abbas to Dubai. All 290 people aboard were killed.
The investigation that followed became one of the most comprehensive analyses of human decision-making under stress ever conducted on a military engagement. The Aegis system had accurately tracked the aircraft as climbing and on a commercial flight corridor. The errors occurred in the human interpretation of that data under extreme psychological pressure: Vincennes was simultaneously engaged with Iranian gunboats, her captain was operating in a combat environment for the first time, and the CIC team filtered the Aegis data through their expectations rather than what the system was actually reporting.
The findings drove fundamental changes in how the Navy trained CIC teams, how Aegis data was displayed, and how commanding officers made engagement decisions under time pressure. The term "scenario fulfillment" , a cognitive bias where decision-makers unconsciously interpret ambiguous data to confirm what they already expect to see , entered the vocabulary of military training because of what happened aboard Vincennes that day.
Named for the Indiana frontier fort where George Rogers Clark secured the Northwest Territory in 1779, Vincennes was decommissioned on 29 June 2005. No collection of American naval history is complete without understanding what happened on 3 July 1988 and what the Navy did about it.
